Academic Honors and Awards Forms
Attention Students:
If you have completed 12 or more credit hours and have maintained a GPA of 3.0 or higher for the Fall 2010 semester, you may be eligible to receive one or more academic honor awards. If you are interested in receiving an award, please read carefully the criteria for each award and fill out the appropriate form.
These forms must be completed by February 28, 2011.
Any forms received after this date will not be considered.
Once submitted, based on your curriculum, the forms will be sent for review by the designated academic department committee. If you are selected you will receive a letter informing you of your award status and instructions on notifying the Student Activities Department of your participation in honors night festivities.
